• Universal Form (Facts for Parents) English    Spanish
• Home Language Survey
• Copy of Birth Certificate, Baptismal Certificate, or Passport (must show parent's name)
• Current verification of residency such as utility bill, rental agreement, mortgage document, or military housing orders (up to 6 months in advance; this does not apply if you are on the housing waitlist).
• Current immunization record. For the most up-to-date requirements, please visit SDUSD Nursing and Wellness / immunization requirements. 
• Copy of transcript, and latest progress report card. You can also complete the Transcript Request Form
• Copy of IEP or 504, if student is currently enrolled in a Special Education Program
